Case Study: #BVM-101

I reviewed one of my patients’ living at Bella Vista Manor medical records in detail. I do have his records from Watertown Regional Medical Center on file.

This very pleasant gentleman had initially been admitted to Watertown Regional Medical Center with weakness, altered mental status, marked dehydration, and multiple other issues. At that time, his medical condition was poor. In fact, his mode of admission to the hospital was paramedics, and firefighters had to break down the door of his home. Then he was transferred to Watertown Medical Center, where he had very comprehensive evaluation.

On review of his records, he was on Glucotrol and also had an order for Humalog for sliding scale insulin.

He was then transferred to an assisted living facility in the Milwaukee area. As you know, he has been a resident now at Bella Vista Manor, Oshkosh, WI for some time. He has had an excellent response to his treatment here.  He has had progressive dimunition in his blood sugars, and his hemoglobin Alc levels have been followed carefully and they are at the goal.

He has tapered off of the medications except for low-dose Glipizide. He is being evaluated with blood sugars at this time, and we are hoping to increase his activity and even reduce his need for diabetic medication in the future.

Overall, he has done extremely well and has been followed very carefully by the staff at Bella Vista Manor. He has had a careful dietitian input, very comprehensive physical therapy, and ongoing nursing care following all of his medical problems.

Thank you to all at Bella Vista Manor in the care of him. As you know, he is also having marked improvement in his mental status. He has gone from being very agitated and paranoid to being quite conversant and pleasant. His family also appreciates all the he has received at Bella Vista Manor.
