
Fall Prevention for Seniors
CRL’s innovative Fall Prevention Assessment and Plan of Care is designed to promote safety by identifying residents who are at risk for falls, and implement an action plan with measurable results including tracking their progress.
Our program is also designed to prevent further injury and/or falls by effectively managing residents by analyzing fall data for root causes, trends, and patterns. From this information we educate our healthcare team and families on measures to prevent falls and promote safety.
CRL’s Fall Prevention Assessment and Plan of Care include:
- Initial Assessment: Residents are assessed by a registered nurse on admission to determine their risk for falling using our proprietary “Fall Risk Assessment” form.
- Falls Reassessment: A fall prevention reassessment is completed in conjunction with the Individualized Service Plan (I.S.P.) and in the event the resident’s condition changes.
- Action Plan: An individualized plan of action for the resident who is at risk will be addressed in the I.S.P. and monitored for any changes if needed.
- Measured Results: As part of our Quality Improvement process we will identify trends and set goals to continually improve our quality outcomes.
